First Page – NOTICE OF INTERMENT (Code I) – Identifies deceased with name, status (married, spinster, or widow

etc), address, age, occupation if any, details of service, grave size, grave location, where service was held. With Still

Born children of which there are many, parent or parents full name and address.

Second Page – NOTICE TO SEXTON (Code S). Similar to some of page 1 but can include circumstance of death i.e.,

‘died on Midland Red bus!’ plus CERTIFICATE FOR BURIAL OR CREMATION. Issued by Registrar. Some

records have additional information (Code M) – sometimes on additional pages there are associated letters from

Minister re. Service or from Undertakers.

When requesting a copy of a register entry please quote: Surname, Forename(s), Year, Cemetery and no of images so

that the correct entry can be supplied by the index holder.
